Filing a DMCA Notice

3. Identification of the material that is claimed to be infringing, with sufficient information to locate it
4. Your contact information (address, telephone number, and email)
5. A statement that you have a good faith belief that the use is not authorized by the copyright owner
6. A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the copyright owner

Counter-Notice

If you believe your content was removed in error, you may file a counter-notice. Counter-notices must include your contact information, identification of the removed material, a statement under penalty of perjury that you believe the material was removed in error, and your consent to jurisdiction of the federal court in your district.
